Palliative Care is a Family Concern
The purpose of palliative care is to provide comfort and relief from pain while maintaining dignity. Qualicare Toronto’s nurses and personal support workers (PSWs) work as a dedicated team, led by a palliative doctor, to help patients and their families with both physical and emotional needs. We provide the extra support needed to improve quality of life.
When a life-threatening illness can no longer be cured, or when attempting a cure will result in lower quality of life than letting it progress, receiving palliative care at home instead of in a hospital or other facility provides many benefits:
- Living in the comfort of your own home
- Taking control of your own care and home environment
- Being surrounded by family and friends
- Continuity of care and caregivers
- Collaborating with CCAC services and your entire healthcare team
- Regular nurse reports and communication at your convenience
- Access to hospital beds and equipment
- Flexible scheduling to suit family needs and manage costs'
Our team of experienced and compassionate nurses and caregivers can help the patient and their family to maintain more of a sense of normalcy during a difficult time. Qualicare provides each client a dedicated Personal Care Manager (PCM), a qualified nurse who will become a trusted confidant, offer support to each family member, and maintain an open dialogue among the patient, family, and healthcare team.
Our PCMs know how to navigate the Ontario healthcare system and will advocate on the family’s behalf to access resources and maximize government-funded services. You can spend more time together instead of stressfully interacting with Ontario’s over-burdened healthcare and social work professionals. When the CCAC is involved, our PCM coordinates with their Palliative Care Nurse Practitioner to minimize your costs and ensure consistent and continuous care. Our top goal is to bring peace of mind and relieve stress for you and your family.
